Robotic Surgery in Sex Reassignment

Robotic vaginoplasty uses the Da Vinci system to create the vaginal canal with peritoneum or with a segment of the sigmoid colon, through small incisions. The peritoneal technique is used in primary surgery, and the intestinal technique in revisions, in patients with limited penile tissue and in cases of previous vaginal stenosis. Robotics is coordinated by Dr. Rinaldo Danesi, and the technique is chosen at the assessment consultation.

Peritoneal Technique

Robotic Peritoneal Vaginoplasty

In robotic peritoneal vaginoplasty, we create the vaginal canal using peritoneal tissue (the membrane that lines the abdominal cavity).

The Da Vinci robotic system gives precise access to the pelvic cavity through small incisions, allowing careful mobilisation of the peritoneum to form the vaginal lining. This technique is particularly suited to patients who would like depth and natural lubrication.

Depth

The technique allows the vaginal canal to be created using peritoneal tissue.

Natural Lubrication

The peritoneum produces natural lubrication.

Robotic Precision

The Da Vinci system offers a magnified 3D view and precise movements in confined spaces.

Small Incisions

Access is through small incisions.

Intestinal Technique

Vaginoplasty with an Intestinal Segment

Robotic vaginoplasty with an intestinal segment uses a portion of the sigmoid colon to create the vaginal canal. This approach is indicated for patients who need revision surgery or who have conditions that limit other techniques.

The robotic system allows precise removal of the intestinal segment and its transposition to form the neovagina, all through minimally invasive incisions. Intestinal tissue naturally produces mucus, which provides continuous lubrication.

Intestinal Tissue

The intestinal segment provides a naturally moist and resilient lining.

Personalised Choice

Which Technique Is Right for You?

The choice between the peritoneal and intestinal techniques depends on a number of individual factors. During the consultation, we assess each case to recommend the appropriate option.

Below is a description of each vaginoplasty technique mentioned on this website. None of them suits every case, and the recommendation comes from an individual assessment.

Peritoneal technique

  • The vaginal canal is lined with peritoneum, the membrane that lines the abdominal cavity
  • Access to the pelvis is through the Da Vinci system, via small incisions in the abdomen
  • Situations in which the team usually considers it: primary reassignment surgery and a wish for vaginal depth, with a preference for subtle natural lubrication

Intestinal technique

  • The vaginal canal is lined with a portion of the sigmoid colon, which produces mucus
  • The segment is removed and transposed using the Da Vinci system, via small incisions in the abdomen
  • Situations in which the team usually considers it: revision or secondary surgery, limited penile tissue, a need for abundant lubrication and cases of previous vaginal stenosis

Penile inversion

  • The skin of the penis is inverted and joined to the scrotal skin to form the inner walls of the neovagina
  • It is the technique the team prefers to recommend
